Hardly anyone comes in because they spotted a new wrinkle that morning. More often the trigger is a photograph of themselves that came as a surprise, or being asked whether they are tired after a weekend that was actually restful. For years it gets postponed with creams and the hope it will pass on its own — and when the moment comes for something more serious, many stop halfway, afraid of the scalpel and wary of devices. That is why the first step with us is not a treatment. The first step is an examination, where you will hear whether 4D laser tightening is the right measure in your case.

What 4D laser tightening of the face, neck and décolletage is
4D laser tightening is a non-invasive treatment that works through the full thickness of the skin, performed with two medical lasers of different wavelengths, Er:YAG and Nd:YAG. The name “4D” comes from the four steps performed in a single session, each targeting a different level — from the deepest layers to the very surface. Unlike ablative laser treatments, here the skin stays intact, so there is no peeling and no dressings.
The first step is performed from inside the oral cavity — from the inner side of the face — and this is what sets the protocol apart from other laser treatments. It sounds unusual when you first hear it, and almost everyone asks about it, but it is a load-bearing part of the protocol. The other three steps are performed from the outside, over the skin of the face, neck, and décolletage.
The goal is for the skin to regain part of the firmness it has lost. In practice, that means a clearer contour, shallower-looking wrinkles, and a more even texture — within the limits of your face.
Who the treatment suits, and who it does not
The treatment makes sense if the laxity is mild to moderate and if the main problem is the quality and tension of the skin. It is most often chosen by people who notice that their jawline has blurred, that the skin of their neck and décolletage looks thinner and slacker, or that photographs keep catching them with a tired expression.
Increasingly, we also see people whose face has slackened after faster weight loss, because the skin could not keep up with the change in volume. That is a real and increasingly common situation, not a rarity no one talks about.
The treatment is not the right choice if the laxity is advanced and there is genuine excess skin. We write about that in more detail further down the page, together with what is worth considering in that situation.
How it is performed
What the treatment looks like, step by step
The treatment takes between 45 and 60 minutes and is performed in a single session, without anesthesia and without preparation that would require you to interrupt or postpone anything. Afterwards you go home and return to your obligations the same day.

The four steps follow one another, and each has its own task:
- From inside the oral cavity, using the non-ablative mode of the Er:YAG laser, the contraction of existing collagen and the formation of new collagen are stimulated. This is also where the nasolabial folds are addressed — from the inner side of the face.
- The Nd:YAG laser works on deeper irregularities in the skin, with the goal of better tone.
- Heating through the skin down to the subcutaneous tissue achieves an additional tightening effect.
- A final pass with the Er:YAG laser works on the very surface and behaves like a very gentle peel.
The hand performing the technique decides the result just as much. The Maletić Polyclinic has worked with lasers since 2008, the year it also became a world education center for laser procedures in cooperation with the Fotona Laser & Health Academy. In practice this means physicians from the United Kingdom, Italy, the United States, Canada, Brazil, Turkey, Israel, Russia and other countries come to Zagreb and Daruvar to learn these methods, and in Daruvar we have a training hall from which procedures are broadcast live. At the clinic we work exclusively with Fotona lasers — devices from the same manufacturer we train on.
Why part of the treatment is performed from inside the mouth
Because it is the shortest route to the deeper layers of the skin. The lining of the oral cavity is thinner than facial skin, so the heat reaches the layer where collagen is produced more easily, without passing through the surface. Studies that compared the same mode of work from the inside and from the outside showed the internal approach to be more effective, with a thickening of the subcutaneous layer that the external approach does not achieve to the same degree.
For you it looks simple: for a short while, the work is done from the inner side of the cheek, with a feeling of warmth. There are no needles and no cutting.
What you feel during the treatment
You feel warmth and mild tingling as the laser passes over the skin. We will not write that the treatment is completely painless, because that would promise something that cannot be guaranteed for everyone. What we can say is that anesthesia is not needed, that the sensation stops when the treatment ends, and that you can tell us to slow down at any moment.

At the Maletić Polyclinic we work exclusively with lasers made by Fotona — a cooperation that dates back to 2008, when we also became its education center. The device, however, is only one part, because the outcome of a procedure is decided by the knowledge with which it is applied.

Daruvar is home to a training hall with live broadcasts of procedures from the operating room, so learning happens on real clinical cases.

Honest about the limits
What you can realistically expect, and what you cannot
This is the part missing from most pages, so we write it without embellishment.
Part of the effect is visible immediately, because the existing collagen contracts under the effect of heat. That impression partly recedes in the first days — an expected course, not a sign that the treatment failed. The real result builds gradually over the following weeks, as the skin produces new collagen. That is why we do not assess the result the next day, but at the follow-up.
The result is not permanent, because the skin keeps aging after the treatment, so the treatment is repeated over time for maintenance. We will not quote a number of years the result lasts, because it differs from person to person — and a rounded figure on a website usually means someone is guessing.
It is also worth knowing how a result like this is measured. Objective measurements of the shift in facial contour after the two-wavelength protocol were taken one week, one month, and three and six months after the last treatment, because the change builds gradually and shows within that range. The professional literature on non-ablative lasers consistently describes the result as gradual and restrained, not dramatic — and that is the level of change we are talking about too. In everyday life it usually means that the photograph taken from the side, and the sight of your own neck on a video call, stop being the first thing your eye lands on. What the treatment does not change is the shape of your face, or the pace at which it continues to age.

When a laser will not replace surgery
A non-ablative laser acts on the quality and tension of the skin, but it does not remove excess skin and does not lift the deeper structures of the face. This is a limit of the method itself, not a shortcoming of the device, and that is how it stands in the professional literature: laser treatment gives a general impression of firmness, while advanced laxity of the facial skin is resolved with surgery.
If at the examination we see that your skin sags, that the contour has been lost to a greater degree, or that there is genuine excess tissue, we will tell you that three laser treatments will not give you what you expect. In that case we talk about a facelift, or we suggest you do nothing and come back in a few years. We have no problem with you leaving the office without a booked appointment.
Sometimes the answer runs the other way: if the main problem is texture, scars, or surface changes rather than laxity, a different treatment is more appropriate and we will point you to laser skin rejuvenation.
When the treatment should be postponed
A few situations call for moving the appointment, not forcing it:
- Tanned skin. A laser is not applied to skin with activated pigment, because the risk of pigmentation changes increases, so we schedule the appointment once the pigment has settled.
- A sun holiday coming up. If you are heading to the seaside in two weeks, we will move the treatment to after your return.
- Isotretinoin within the last year. Tell us if you have taken it, because it affects how the skin reacts.
- An autoimmune disease, weakened immunity, or an active infection.
- A tendency to cold sores because part of the treatment is performed inside the oral cavity.
- A skin lesion in the treatment area that first needs to be examined.
Most of this does not mean a ban — just a different plan and a different date.
After the treatment
Recovery and care after the treatment
No downtime is needed, and you return to your usual obligations the same day. After the treatment, mild redness and a feeling of warmth are possible, which as a rule subside within a few hours. With more sensitive skin the redness can last somewhat longer, and that is expected.
What really matters in the days after the treatment comes down to two things: sun protection and patience. Use protection consistently, not just when it is sunny, because after a laser treatment the skin is more sensitive to ultraviolet radiation and that is when the risk of unwanted pigmentation is highest. You will receive written care instructions before you leave, so there is nothing you need to memorize.

Who performs the treatment at the Maletić Polyclinic
The treatment is performed by Ines Maletić, MD, co-founder of the Polyclinic and one of the two physicians who opened it in Daruvar in 1998. Alongside her specialization in anesthesiology and reanimatology and further training in the Czech Republic and Austria, she is an Expert Clinical Lecturer at the Laser & Health Academy — a physician who not only performs laser methods but teaches them to colleagues from abroad.

The price of 4D laser tightening

Does 4D laser tightening hurt?
The treatment is performed without anesthesia and most people describe it as comfortable, with a feeling of warmth and mild tingling as the laser passes over the skin. We will not promise you that you feel nothing, because that would not be true — but the sensation is mild and stops as soon as the treatment ends. If you are sensitive to heat, tell us right away and we will adjust the pace.
How many treatments are needed, and when are results visible?
With us, the protocol usually consists of three treatments at agreed intervals, and the exact schedule is set according to the condition of your skin. Part of the effect is visible immediately after the first treatment, because the existing collagen contracts under the effect of heat. The rest builds gradually over the following weeks, as the skin produces new collagen, so a true impression of the result forms only after several weeks.
How long do the results last?
The result is not permanent, because the skin keeps aging after the treatment. How long it lasts depends on your age, genetics, sun exposure, and habits, so we will not quote a number of years that would sound convincing but would not hold true for everyone. Most people repeat the treatment over time for maintenance, and at the follow-up we will tell you when that makes sense.
Can a 4D laser replace a facelift?
No — and that is the most important sentence on this page. The laser acts on the quality and tension of the skin, but it does not remove excess skin and does not lift the deeper structures of the face. With mild to moderate laxity the result can be very satisfying, while with advanced laxity a laser cannot deliver what surgery can. If we judge that you are in the second group, we will tell you so at the examination.
Can the treatment be done in summer?
In principle yes, because this is a non-ablative treatment that does not break the skin. What really matters is not the season but the condition of the skin: a laser is not applied to tanned skin, and after the treatment consistent sun protection is required. If you are planning a seaside holiday in two weeks, we would rather move the appointment than risk a change in pigmentation.

Because that route reaches the deeper layers of the skin more directly than from the outside of the face. The lining of the oral cavity is thinner than skin, so the heat reaches the level where collagen is produced more easily, especially in the area of the nasolabial folds. This part of the treatment is performed with a special non-ablative mode and does not damage the mucosa.
Who should not have this treatment?
We postpone the treatment if the skin is tanned or if you are planning sun exposure soon. Caution is also needed if you have taken isotretinoin within the last year, if you have an autoimmune disease or weakened immunity, an active infection or a skin lesion in the treatment area, or if you are prone to cold sores. Tell us all of this at the examination, because part of it does not mean a ban — just a different plan.
