Why the mons pubis becomes enlarged

The mons pubis is a natural pad of fat tissue above the pubic bone. Its size differs from woman to woman and can change with age, pregnancy, hormonal shifts, and changes in body weight.

Mons pubis reduction is considered when the area is more pronounced than the patient would like, or when it causes functional discomfort — friction, irritation, or difficult hygiene, for example.

After childbirth and a cesarean section

Pregnancy stretches the skin and changes the distribution of fat tissue. After a cesarean section, the scar can pull in the tissue above it, so the area looks more prominent than before, even though the amount of fat tissue has not substantially changed. This relationship between the scar, the skin, and the subcutaneous tissue can create a more visible bulge above the scar line.

After weight changes

With weight gain, fat tissue accumulates here too, and the skin stretches. If that has gone on for a longer time, the skin may not return to its previous state after weight loss. That is why the area can remain comparatively more prominent than the surrounding parts of the body after losing weight, especially if there is also excess skin.

An honest assessment

When mons pubis liposuction may not be appropriate

When there is suspicion that the bulge is not fat tissue. A firm or changeable bulge that becomes more pronounced when coughing or straining requires surgical work-up before any aesthetic procedure, because it may point to a hernia or another condition. Further treatment is determined once the diagnosis has been made.

When you expect it to solve the abdomen as well. This procedure changes one area and does not close the subject of the abdomen. If the main problem is lax abdominal skin, the solution is a different procedure, not this one.

When your weight is still changing. The result lasts with a stable weight. If you are planning major weight loss or your weight fluctuates, it is better to wait, because the appearance of the area would change again afterwards.

When pregnancy is planned in the near future. Pregnancy can change the skin and the distribution of fat tissue and can diminish or alter the result of the procedure. That is why the patient’s reproductive plans should be taken into account when scheduling.

If you smoke. Smoking increases the risk of delayed healing, infection, and wound problems. Before the procedure, stopping or temporarily pausing smoking needs to be discussed, following medical guidance.

The examination can end with a recommendation for liposuction, a different procedure, further work-up, or the conclusion that an operation is not indicated. The decision depends on the cause of the bulge, the quality of the skin, your expectations, and the possible risks.

Examination and skin assessment

At the examination we assess the amount of fat tissue, the quality and tightness of the skin, and the relationship to the lower abdomen. That is when it is decided whether liposuction is enough or whether a procedure that also removes skin is needed. If there is any suspicion that the bulge is not fat tissue, that is clarified before anything else.

The procedure and the compression garment

Localized fat tissue can be removed by liposuction. At the Polyclinic, laser-assisted liposuction may be used, subject to the operating surgeon’s assessment. Any effect of the method on skin retraction depends on the skin’s initial quality and cannot replace the removal of pronounced excess skin.

A standalone procedure is most often performed under local tumescent anesthesia, while general anesthesia may be used for more extensive combined procedures. Small scars remain at the cannula insertion points, usually a few millimeters long. After the procedure, a compression garment may be recommended to support the area and control swelling.

Recovery after the procedure

• The first few days — swelling, sensitivity, and bruising are usually at their most pronounced. The compression garment is worn as instructed by the surgeon.
• The first week — returning to lighter daily obligations and desk work is often possible within a few days, depending on the extent of the procedure.
• Approximately 2–3 weeks — the compression garment may continue to be worn as recommended, to support the area and reduce swelling.
• The following weeks — the swelling gradually subsides and the contour of the area becomes clearer.
• Returning to sport and heavier exertion — determined by the course of healing and your physician’s assessment.

After recovery

What you see once the swelling has gone

The final contour is assessed after several months, once most of the swelling has subsided and the tissue has stabilized. The goal is to reduce the prominence of the area and achieve a more harmonious transition toward the lower abdomen, but the extent of the change depends on the initial anatomy, the skin, and healing.

In published series, patients report easier hygiene, less irritation in the fold, and less discomfort when moving and wearing clothes. Such outcomes are not the same for everyone and depend on the initial symptoms and the type of procedure.

The procedure changes the contour of one area, and that is where its limit lies: it does not change body weight, the shape of the abdomen, or the quality of the skin above it.
