Procedures on the male genitalia cover a range of medical and aesthetic indications, from phimosis and the consequences of losing a testicle to dissatisfaction with size. The examination serves to distinguish normal anatomy, a functional problem, and a condition that requires surgical or other treatment.

When men come for an examination

The most common reasons fall into three groups, although they may overlap in an individual patient.

  • Recurring or functional problems. Foreskin infections, tearing or bleeding during intercourse, difficulty retracting the foreskin and maintaining hygiene, pain during erection or inability to have intercourse.
  • An absent or undeveloped testicle. A testicle absent after surgery for a tumor, torsion or injury, as well as a testicle that did not develop or has atrophied.
  • Appearance and size. Dissatisfaction with the length, girth or appearance of the genitalia, where an objective examination and measurement are needed to assess the anatomical findings.

All three groups are a valid reason to come for an examination. That, however, does not mean surgery is needed.

Honesty first

Why the examination comes before the decision

The examination establishes the cause of the symptoms, the anatomical findings, the medical indication and your expectations of the procedure. Only then can we assess whether an operation is appropriate and what it can achieve.

When the concern is size, an objective assessment matters. Stretched length is measured from the pubic bone to the tip of the glans, which reduces the influence of the suprapubic fat pad on the result. A normal anatomical finding does not dismiss dissatisfaction, but it significantly affects the assessment of the indication and the expected benefit of surgery.

When the foreskin cannot be retracted, the assessment depends on age and the type of change. In boys, a non-retractable foreskin is often a normal stage of development, and for symptomatic phimosis without scarring, European urological guidelines list a topical corticosteroid preparation as the first line of treatment. In adults, the change is more often scar tissue, where circumcision can remove the narrowed tissue and the problems associated with it.

Implanting a testicular prosthesis is not a medical necessity but the patient’s personal choice. Before the decision, it is important to explain that the prosthesis replaces volume, but is firmer to the touch than a natural testicle and may differ in size and position. In studies, these properties are among the most common patient complaints.

Procedures on the male genitalia at the Maletić Polyclinic are performed by Duško Maletić, MD, MSc, a specialist in surgery since 1984 and in urology since 1988, and co-founder of the Polyclinic. Both specializations matter when assessing the aesthetic and functional aspects of these procedures. He has performed surgery in this anatomical region since the Polyclinic was founded in 1998.

Penis enlargement comprises lengthening by dividing the suspensory ligament and thickening with your own fat tissue. Lengthening primarily changes the appearance in the flaccid state, while with thickening, part of the transferred fat tissue may be resorbed and an additional stage may be needed.

Circumcision is performed in adults for scarring phimosis, recurring infections, or hygienic, religious or aesthetic reasons, and in children when there is a medical indication.

Testicular prosthesis implantation replaces the volume of an absent testicle after its removal or when the testicle did not develop. A prosthesis can also be implanted years after a previous operation, if the condition of the tissue and your health findings allow it.

There is no need to decide on your own, before the examination, which procedure would be appropriate. The examination establishes the cause of the symptoms and assesses whether surgical treatment is indicated at all.

After the examination

What each procedure can change

The possible change depends on the initial problem and the type of procedure. With phimosis, the goal is to remove the problems associated with the narrowed foreskin; with a prosthesis, to replace the volume of the absent testicle; and with lengthening or thickening, to change certain dimensions of the genitalia within the limits of the method.

  • with scarring phimosis, the problems associated with tearing, infections, difficult hygiene and painful erections can be reduced or removed
  • a testicular prosthesis can replace the volume of an absent testicle and reduce visible asymmetry, but it does not restore the testicle’s function
  • lengthening changes primarily the appearance of the penis in the flaccid state, while the change during erection is small or absent
  • thickening increases girth with your own fat tissue, with partial resorption to be expected and a possible need for an additional stage
  • the examination may end with a recommendation for surgery, another form of treatment, or the conclusion that an operation is not indicated

The outcome is not the same for everyone and depends on the initial findings, the type of procedure, healing and expectations. In men circumcised for phimosis, prospective studies record a reduction in itching, burning and pain during intercourse, while the professional literature describes a better body image in a proportion of patients after testicular prosthesis implantation. None of these procedures can change the experience of illness, injury or previous surgery, and aesthetic procedures cannot guarantee a changed relationship with your own body.
