What a scalp filler is and what it contains
A scalp filler is an injectable therapy for weakened, thin hair. It delivers a complex of seven peptides and hyaluronic acid into the scalp skin — substances that stimulate follicle activity and improve the condition of the scalp. It is used equally in women and men, most often for increased shedding and thinning hair.
This is a medical treatment, not cosmetic care. That is why an examination comes first, and the therapy is recommended only when the findings show that the follicles are still active and the therapy can have an effect.
Why it is called a “filler” when it adds no volume
The name can be confusing, because most people associate fillers with lips and the face. There, a filler really does add volume beneath the skin. A scalp filler does something entirely different. It does not shape or fill tissue — it delivers active substances into the layer of skin where the follicles sit, with the goal of strengthening the existing hair and improving its density. What they share is the method of application and the name, but not the purpose of the treatment.
What it acts on
In the scalp, peptides and hyaluronic acid act on several processes at once. According to the available data, peptides and hyaluronic acid can support follicle activity, act as antioxidants, and stimulate angiogenesis — the formation of new blood vessels — which improves the scalp’s blood supply. Better blood supply can contribute to the delivery of oxygen and nutrients the follicles need to work normally.
The filler, then, does not create new hair and does not add volume. Its goal is to improve the conditions in which the existing follicles function.

Who a scalp filler helps the most
The people who can benefit most are those whose thinning is still in an earlier phase. When the hair has thinned but the follicles are still active, the therapy can act on the existing hair. The more advanced the hair loss, the more limited the expectations from the therapy.
Situations in which we suggest it

- Increased shedding and early thinning, when density is dropping and the part line is slowly widening.
- Diffuse thinning in women — an even loss of density across the entire scalp.

- As a complement to other scalp therapies, PRP for example, when the findings show they complement each other well.
Filler, PRP, or mesotherapy: how the therapy is chosen
This is the most common question at the consultation, and understandably so, because all three therapies deliver active substances into the scalp — they differ in what is applied. A PRP treatment uses your own plasma, mesotherapy uses nutrients composed according to your findings, and the filler uses a complex of peptides and hyaluronic acid.
The therapy is not chosen by how popular a treatment is, but by the findings and measurements of the scalp. Some of the methods can be combined when the findings show a need to improve the condition of the scalp and act on weakened follicles at the same time.

1 · Examination and TrichoLAB scalp analysis. We examine the scalp trichoscopically, and when a more precise picture is needed, we use the TrichoLAB system, which measures hair shaft thickness, density, and the number of hairs per region. From those numbers we can see what kind of hair loss is involved and whether the filler has anything to work on.
2 · Preparation. The area of the scalp being treated is disinfected with an antiseptic. If you are more sensitive, local anesthesia can be applied by agreement.
3 · Application. We apply the filler with fine needles into the areas determined by your findings, and the scalp is then gently massaged so the preparation spreads evenly.
4 · After the treatment. You return to your routine right away, with no downtime. You receive instructions for the following days in writing.
The treatment is usually performed as a series of four treatments over eight weeks, roughly two weeks apart. After the series we measure the scalp again, so the changes can be assessed by comparing objective measurements with the initial findings.

What the filler cannot do and when we do not recommend it
This part of the conversation happens at the first examination, before a single treatment is agreed.
It does not create new follicles. Where there are no follicles left, injectable therapies cannot restore lost hair, so the filler is not a solution for a bald crown or for areas with no hair. In that situation it is more appropriate to consider a hair transplant than to go through a therapy with no expected benefit. See hair transplant.
It does not remove the cause of hair loss. If the hair loss is linked to a thyroid disorder, iron deficiency, or a hormonal change, the underlying cause is treated first. No scalp treatment can replace the necessary diagnostics and treatment of the underlying condition.
Before-and-after photographs cannot predict the result in every patient. They show results in patients for whom the therapy was indicated, but the result depends on the state of the scalp and the cause of the hair loss. That is why we measure first, and only then talk about what is realistic to expect in your case.
If the findings do not point to an expected benefit from the filler, we will not recommend the therapy. We suggest a series of treatments only when there is a clear professional indication for it.
A realistic expectation
What actually changes after the series
The scalp does not change quickly, because the therapy acts on the hair growth cycle — and that cycle is slow. In clinical studies of the peptide and hyaluronic acid complex, a series of four treatments over eight weeks is evaluated only six months after the start of therapy, and even with standard medications for hereditary hair loss, dermatologists make their first serious assessments after six to twelve months. What can be confirmed in that period is greater hair shaft thickness, greater density, and less shedding.
In everyday life the change is smaller than people expect. It usually comes down to less hair on the brush, a part line that no longer needs combing over before going out, and a photo under ceiling light that stops being a reason to look away.
The filler does not change the cause of the thinning, so the achieved condition is maintained only while the therapy continues.

Who will be taking care of you
The Hair Center is led by Ana Maletić, MD, PhD, a specialist in plastic, reconstructive, and aesthetic surgery. She holds the FISHRS and ABHRS Diplomate titles, is the only physician in Croatia with the ABHRS Diplomate title and one of 260 physicians worldwide to have earned it, and is the only physician from Croatia in regular membership of the ISHRS. In March 2026 she defended her doctoral thesis on the connection between hair transplantation and biopsychosocial factors of health.
The treatment is performed by a physician who deals with the diagnostics and treatment of hair loss every day, and who assesses whether the scalp still has active follicles the therapy can act on. One hair filler treatment costs €335.00, and a package of four treatments €1,100.00. The trichoscopic scalp analysis costs €135.00, and its findings determine whether we will suggest the therapy at all.
The consultation is free of charge. At the examination we assess the condition of your scalp and explain the findings to you — including the possibility that the filler is not indicated in your case. Which method for whom
Patients most often ask us which hair therapy is the best — and the answer is that a universally best therapy does not exist. Each person’s hair loss has its own cause, phase, scalp condition, and degree of follicle preservation. That is why the same method gives a good result in one person and does not match the findings in another.
So we build the treatment plan around what is actually measured on your scalp. After the trichoscopic analysis we know what kind of hair loss is involved, how far the process has advanced, and whether there are still follicles with regenerative potential. From those findings comes one therapy, or a combination of methods that complement each other. Sometimes what comes out is that a treatment makes no sense in your case — and then we talk about a transplant or about monitoring, depending on the findings. You will hear that at the first examination, not after several paid treatments.

The methods are often combined, because the same scalp can simultaneously hold weakened follicles and areas where hair no longer grows. Then one method preserves and strengthens what is left, while the other replaces what has been permanently lost.
It is just as important to know what these methods cannot do. No therapy creates new follicles, so where a follicle is permanently lost, no number of treatments can change that. Nor does any of them remove the cause of hair loss. If a thyroid condition, a nutrient deficiency, or a hormonal change is behind the shedding, that finding is treated separately, and scalp therapy goes alongside it — not instead of it.
The right method for you is the one that matches the state of your scalp, regardless of how popular it currently is. That is why every plan starts with an analysis and a conversation — and only then comes the choice of treatment.

Is a scalp filler the same as a lip or facial filler?
It is not, although the name is confusing. A facial filler adds volume beneath the skin, while a scalp filler shapes nothing — it delivers peptides and hyaluronic acid into the scalp skin to stimulate follicle activity. The goal is not to fill space but to strengthen the hair and slow its shedding.
Does a scalp filler treatment hurt?
You do feel it, but briefly. The filler is applied with very fine needles into the surface layer of the scalp skin, and after application the area is gently massaged. Anesthesia is usually not needed, but local anesthesia can be applied by agreement if you are more sensitive.
How many scalp filler treatments are needed?
The usual recommendation is four treatments over eight weeks, that is, roughly two weeks apart. In that case it is more useful to consider a hair transplant than to go through a series of treatments with no expected benefit.
